The 6 plastic circles have been carefully removed with a coin or key. The cans were then drunk one by one, holding the entire six-pack above the head. The pitfall, however, was sometimes forgetting which can had been opened. The result is a whole load of beer or soft drink down your neck. Not very appetizing.
